New Hampshire State law allows a licenced medical provider to certify that a patient has a qualifying medical condition.  The required Written Certification is not intended to be a prescription or medical recommendation for the therapeutic use of cannabis.

  •  A certification is completed for submission to the State of New Hampshire after completing a physical evaluation and review of records 

  • The state application process takes up to 20 calendar days from the date a complete application is received by the Therapeutic Cannabis Program.

 

  • You will then use your Registry Identification Card to receive services from your designated Alternative Treatment Center.

Qualifying Conditions

  • Cancer

  • Glaucoma

  • Positive status for human immunodeficiency virus

  • Acquired immune deficiency syndrome

  • Hepatitis C

  • Elevated intraocular pressure

  • Cachexia

  • Chemotherapy-induced anorexia

  • ​​Moderate to severe vomiting

  • Seizures

  • Severe, persistent muscle spasms

Severe pain that has not responded to previously prescribed medication

or surgical measures or for which other treatment options produced serious side effects

  • Amyotrophic lateral sclerosis

  • Muscular dystrophy

  • Crohn's disease

  • Multiple sclerosis

  • Chronic pancreatitis

  • Spinal cord injury or disease

  • Traumatic brain injury

  • Epilepsy

  • Lupus

  • Parkinson's disease

  • Alzheimer's disease

  • Ulcerative colitis

  • Ehlers-Danlos syndrome

OR

One or more injuries or conditions resulting in one or more qualifying symptoms 

  • Wasting syndrome

  • Agitation of Alzheimer's disease

  • Constant or severe nausea

OR

Moderate to severe chronic pain 

OR

Moderate or severe post-traumatic stress disorder
